FEDERAL INVESTIGATORS HAVE RELEASED INFORMATION ON THE DEATH OF A BIG STONE GAP VIRGINIA COAL MINER THAT WAS KILLED EARLIER THIS YEAR. AUTHORITIES SAY AN ERROR IN BOLTING THE ROOF OF A MINE AT OSAKA MINE #1 IS BLAMED ON THE DEATH OF 61 YEAR OLD DAVID SIZEMORE THIS PAST APRIL. THEY CONCLUDED THAT PRECAUTIONS WEREN’T TAKEN, CAUSING A BOLT TO BREAK DURING INSTALLATION. IT’S UNCLEAR WHAT STEPS FEDERAL AUTHORITIES WILL TAKE TO MAKE SURE THE COMPANY IMPROVES ITS SAFETY TO ENSURE THIS DOESN’T EVER HAPPEN AGAIN.
