State documents show the owner of a southwest Virginia coal mine where a worked was killed Tuesday had been cited 48 times for safety violations since 2005. The Richmond Times-Dispatch reports that violations at the Wise County mine included seven for failure to follow a plan to keep the mine roof safe. The documents are from the Virginia Department of Mines, Minerals and Energy. Most of the 48 safety violations were for failures to complete paperwork. The roof fall on Tuesday killed a veteran roof bolter, 61-year-old David Sizemore of Big Stone Gap, at Osaka Mining Corporation’s Mine No. 1. Another miner was treated for injuries. Virginia mining department spokesman Mike Abbott says that state and federal investigators are trying to determine the cause of the roof collapse. Work at the mine has stopped until the investigation is finished. Abbott says the mine’s safety score declined this year, and the state agency had planned to submit the mine to extra scrutiny. Osaka official Hagy Barnett declined to comment on the accident.
